Urząd Stanu Cywilnego in Tarnów (Office of Vital Records) maintains vital records for the city of Tarnów and gmina Tarnów (for localities: Biała, Błonie, Jodłówka-Wałki, Koszyce Małe, Koszyce Wielkie, Lękawka, Nowodworze, Poręba Radlna, Radlna, Tarnowiec, Wola Rzędzińska, Zawada and Zbylitowska Góra). The only accepted way of applying for any document is through traditional mail, written in Polish.
